I was treated with 2 cycles of “hyper CVAD” chemotherapy with central nervous system prophylaxis. Hyper-CVAD is a common chemotherapy regimen. The agents in the regimen are listed below. After receiving this regimen, you underwent autologous stem cell transplant on September 8, 2004.

September 22, 2004: Hickman removal by R. David Sanders, PA-C at City of Hope
AUTOLOGOUS STEM CELL TRANSPLANTATION:
I received autologous hematopoietic stem cell transplantation as part of my treatment for lymphoblastic lymphoma. In autologous stem cell transplantation, my peripheral blood stem cells were collected and then reinfused back into my body after I was treated with chemotherapy. My infusion date was September 8, 2004. To prepare my body for the transplant (conditioning), I received BCNU (carmustine) and etoposide (VP-16). The doses are included in the chemotherapy table above.
TREATMENT COMPLICATIONS:
Chemotherapy was discontinued because of a decrease in my cardiac function.
BLOOD TRANSFUSIONS:
I received multiple blood transfusions during treatment.
